## Position Description:
Home health service is the component of comprehensive health care whereby services are provided to individuals and families in their places of residence for the purpose of promoting, maintaining, or restoring health, or minimizing the effect of illness or disability.
Provides skilled nursing care, under the direction of a physician's plan of care, in a patient's own home to maximize their functions through the use of nursing interventions. Staff nurses serve as case managers, directing and coordinating an interdisciplinary team. Demonstrates strong interpersonal and communication skills with clients, their family and colleagues.

## Required Qualifications:
2 positions available
RN with 1-2 years acute care Med/Surg experience. Current RN license in Maine, driver's license, CPR Card (Healthcare Provider) and a reliable car. Weekend and holiday rotations.
